How to Access a Support at Home Package

Together we can find out if you are eligible for a Support at Home Package, by calling My Aged Care on 1800 200 422 or visiting myagedcare.gov.au

The My Aged Care contact Centre will ask a series of questions to determine if you need an assessment by an Assessment Team.

If this is the case, then the Assessment Team will make an appointment with you for an in-home face-to-face assessment. During the assessment the assessor will determine if you are eligible for a Support at Home Package and which package level best meets your care needs.

There are 4 different levels of Support at Home Package available and each level provides a different level of funding.

When you have been approved for a Support at Home Package, you will enter the national waitlist to get a Support at Home Package assigned.

Once you get a package assigned you will receive a letter from My Aged Care with a referral code and a date by which you must sign up with a provider to receive services. This can be extended once by 28 days if you need more time to decide.

You will need to give this referral code to the Service Provider so they can access your approval.

The time to wait for a Home Care Package can be very long. We recommend following up during the wait time with my aged care.
